Vision 2035 – Health and Wellbeing Board objectives

The Health and Wellbeing Board forms part of the delivery mechanism for the County Durham Vision 2035, with a remit to deliver the following vision objectives and also work with other partnerships on shared priorities and cross-cutting issues.

Along with its subgroups, it takes the lead on delivering the following Vision objectives:

  • improve health and wellbeing outcomes for children and young people
  • help children and families maintain their optimum mental health and wellbeing
  • high quality nursery and primary education
  • Support to expectant mothers and parents of newborn babies and their children
  • High quality support to families and children and young people with special educational needs and/or disabilities (SEND)
  • preparation for adulthood and independence for children and young people with SEND
  • protect and support children and young people in need (including vulnerable children, children looked after, care leavers, young people who have offended and young victims of crime)
  • support services to vulnerable children will be improved
  • create a smoke-free county
  • tackle the stigma and discrimination relating to poor mental health
  • build more resilient communities and promote positive mental health
  • better integration of health and social care services
  • develop care options specifically to meet the needs of older and disabled people
  • develop communities to suit the needs of an older population
